A Brief Overview

The Transplant Nurse Coordinator is involved in general program and patient activities. This Registered Nurse establishes and promotes a liaison relationship between the transplant team members and the patient's families. He/she provides program information, patient follow-up plans, and education related to transplant to patients, families, referring physicians, and nursing staff.

As a liaison to governmental and other regulatory agencies (UNOS, Medicaid, Medicare, etc.), this person will provide information regarding transplant candidates, and transplant patients as required.

What you will do Coordination of transplants during business hours daily.

Acts as liaison between the transplant team and the patient families. Participate in education to families post-transplant in preparation for discharge.

Participate in Transplant Evaluations, including coordinating collection of data.

Responsible for regulatory policies, processes and data entry through UNOS.

Maintain transplant databases, participate in transplant research projects.

Triage phone calls and assist providers in implementing care plans.

• Education Qualifications Bachelor's Degree Nursing Required
• Experience Qualifications At least two (2) years RN experience in pediatrics, critical care, and/or transplant. Required
• Skills and Abilities Ability to provide basic nursing care to children and families in the cardiac transplant program. (Required proficiency)
• Ability to communicate verbally and in writing English. (Required proficiency)
• Critical analysis and problem-solving skills. (Required proficiency)
• Ability to work independently. (Required proficiency)
• Ability to work collaboratively through consultation with other health care team providers. (Required proficiency)
• Licenses and Certifications Nursing Temporary Practice Permit (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or
• Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or
• Registered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire - Required and

Basic Life Support (BLS) - Issued through on organization that requires a hands-on instructor lead psychomotor skills verification (ex. American Heart Association or Red Cross). - within 3 months - Required
