Home Health Occupational Therapist

About the Role

As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes, using creativity, clinical expertise, and problem-solving to help them rebuild the skills needed for everyday living: dressing, bathing, cooking, managing routines, and engaging in meaningful activities.  Your treatment will help patients regain the ability to care for themselves and remain in their own homes.  You will build and execute the care plan, guiding improvements and see the impact of your work on patients' quality of life.

This role is ideal for a licensed Occupational Therapist who thrives in a flexible, patient-centered environment and enjoys tailoring care to each individual's unique goals and home setting. You'll assess functional abilities, design personalized treatment plans, and develop adaptive strategies and solutions that support independence and safety.

Working closely with patients, families, and an interdisciplinary care team, you'll provide education, hands-on training, and encouragement as patients progress toward greater confidence and independence in their daily lives.

Location

• Office Location: Saginaw
• Main Service Area:  Bay County, Midland, Clare, Gladwin, Arenac
• This role requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

Full Time, Monday to Friday, 8 a.m. to 5 p.m.

• Required Qualifications
• Graduate of an accredited occupational therapy program (ACOTE accredited)
• Current state license as an Occupational Therapist
• Minimum 1 year of experience as a licensed Occupational Therapist
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage
• Willingness to travel daily for home visits within service area

Preferred Qualifications

• Home health experience
• Experience with adaptive equipment and home modifications
• Knowledge of Medicare home health documentation

• Knowledge of medical terminology and home health regulations
• Excellent problem-solving and creative thinking skills
• Strong verbal and written communication abilities
• Ability to work independently with minimal supervision

• Interpersonal skills for professional interaction with patients, families, and teams

Pay Range

• $84,000 — $89,000 USD
