RN Hospice Clinical Liaison

We are currently hiring a full-time RN Hospice Clinical Liaison to join our team. This is a hospital-focused liaison role and not a traditional bedside nursing position.
Position Responsibilities:
Develop and maintain relationships with hospital referral sources and healthcare professionals.
Makes regular, planned calls to secure new referrals, build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers.
Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
Requirements
- · Active, unencumbered Registered Nurse (RN) license.
- · Hospital bedside nursing background, including medical-surgical (med-surg) experience.
- · Minimum 1 year of healthcare sales or marketing experience.
- · Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
- · Valid driver’s license, auto insurance and dependable transportation.
