What You Will Do
- Practice independently in the community as part of an interdisciplinary care team.
- Deliver personalized nursing and care management to patients in their home or care facilities.
- Constantly evaluate evolving patient needs and respond with plan of care adjustments.
Qualifications
- Licenses and Certifications: A license and current registration to practice as a Registered Professional Nurse in New York State required and
- Valid driver's license required and
- OASIS certification preferred
- For Hospice Only: Wound Care Certification required or willingness to obtain within one (1) year of hire required
- Certification in Hospice and Palliative Care Nursing required
- Education: Bachelor's Degree in Nursing OR Associate’s degree in Nursing required
- Work Experience: Four years’ clinical home care experience required and
- Minimum of two years full-time clinical home care experience required and
- Minimum of one year of experience as a field staff nurse in VNS Health Home Care, which includes ability to manage/document cases with a minimum of supervision, identify/assess/formulate/implement action plan to address problem and evaluate outcome preferred and
- Joint visits with other nursing staff to demonstrate new techniques preferred
- Hospice Only: four years' clinical hospice experience or clinical home care experience
- Hospice Only: Minimum of two years' full-time clinical hospice or home care experience
Pay Range
USD $126,413.00 - USD $160,188.00 /Yr.
