How it works

We partner with local assisted living communities and adult homes, and our experienced NPs care for their residents through your practice.

Our team and AI platform run your Medicare care management programs (CCM, APCM and PCM), plus scheduling, documentation and billing.

You stay the owner. Everything bills through your clinic, with no new staff and no new overhead.

Requirements

• You're a fit if you
• Are an MD/DO who owns a Medicare-certified primary care clinic (1–2 locations) in Nassau or western Suffolk County
• Have Medicare set up as payee and hold an active New York license and DEA registration
• Are board certified or eligible in Internal Medicine, Family Medicine or Geriatrics
