Hospice Nurse Liaison

For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Qualifications:

• Current New York State Registered Nurse License/Permit.
• Current valid NYS Driver’s License preferred.
• Must have own car to conduct site assessments.
• Minimum of three years relevant clinical experience.
• Working knowledge of inpatient/out-patient/home care/hospice; application of admission/continued stay criteria and third party payers.
• Education: Bachelor’s degree preferred.

Responsibilities:

• The Hospice Nurse Liaison are responsible for coordinating applications through the admission review process. Coordinates and maintains referrer relationships with assigned institutions/agencies. Participates in development and marketing Calvary Hospital’s Home Care and Hospice Services. Provides pre-admission assistance to patients, families and referrers.
