Role Overview
We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents in Eureka and the surrounding Humboldt County area.
What You’ll Do
- Routine & urgent assessments; chronic disease management
- Post-discharge follow-ups, med reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same-day documentation; CMS/regulatory compliance
- Schedule & Setting
- Part-time position with a structured weekday schedule. You’ll spend 3 days per week delivering in-person care across Eureka and the surrounding area, with strategically clustered routes designed to minimize travel and maximize time with patients. Occasional telehealth visits add flexibility to your week.
Requirements
- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility-based experience preferred; EMR proficiency
Compensation
Compensation
- Annual compensation range: $115,000–$130,000 (based on experience, schedule, and patient volume)
- Additional earning opportunities available based on coverage and panel needs
- W-2 with mileage reimbursed per policy
- Final terms outlined in the employment agreement
