Social Worker - Mental Health and Learning Disabilities

Liquid Personnel is recruiting an Adult’s

Social Worker to join its client’s Mental Health and Learning Disabilities team, based in North Lincolnshire.

What will your responsibilities be?

Manage a caseload within the Mental Health and Learning Disabilities Team, completing assessments, care planning, and reviews for adults with complex needs.

Coordinate hospital discharges and Section 117 aftercare planning, working closely with health professionals, service users, families, and partner agencies to ensure safe and timely transitions.

Undertake safeguarding enquiries under Section 42 of the Care Act, assessing risk, leading investigations, and implementing appropriate protection plans and outcomes.

Qualifications and Experience:

• To be successful in this role you must have; Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
